Requesting Records
The Children’s Aid Society of Oxford County provides access to information and disclosure to records pursuant to Part X of the Child, Youth and Family Services Act and the Access to Adoption Records Act.

Access to Personal Information or Disclosure Requests
If you are currently receiving services from Oxford CAS or have in the past, and would like access to your personal information, please complete our Access to Information/Disclosure form (*form opens in new window and submitted on website).
You must attach one copy of valid government-issued photo identification along with your request, and a photograph of you holding your government-issued photo identification. Alternatively, please attach one copy of valid government-issued photo identification along with your request, and a brief virtual meeting or in-person meeting will be scheduled prior to releasing the records to you.

Please note, the written consent of any person age 16 years and older is required to release their information to you. If you wish to obtain the personal information of any person aged 16 years or older, please email disclosure@casoxford.on.ca and request a Consent to Disclose Information form.
A person who is a Substitute Decision-Maker for a child under the age of 16 may request personal information of that child, subject to some exceptions. In some circumstances, you may be asked to provide documents confirming your authority to make the request in place of the child.
Child Welfare Record Check
If you require a Child Welfare Record Check for employment, please fill out the Child Welfare Record Check form (*form opens in new window and submitted on website).
You must attach one copy of valid government issued photo identification along with your request.
Adoption Disclosure Requests
If you were adopted through Oxford CAS and you would like to request non-identifying information, please complete the Adoption Disclosure form.
If you are related to someone who was adopted through Oxford CAS (specifically, if you are the adoptive parent, birth parent, birth grandparent, birth sibling aged 18 or older, or sibling of a birth parent aged 18 or older) and you would like to request non-identifying information related to an adoption, please complete the Adoption Disclosure form.
If you are 18 years or older and the child of an adoptee who is deceased, you may request non-identifying information related to an adoption. Please complete the Adoption Disclosure form and attach a death certificate or other proof of death, and documentation confirming that you are the child of the deceased adoptee.
You must attach one copy of valid government-issued photo identification along with your request, and a photograph of you holding your government-issued photo identification. Alternatively, please attach one copy of valid government-issued photo identification along with your request, and a brief virtual meeting or in-person meeting will be scheduled prior to releasing your records to you.
For copies of adoption orders for all adoptees, or unredacted copies of birth registration, you must apply to the Ministry of Community and Social Services at Service Ontario or toll free at 1-800-461-2156.
Record Correction
If you have previously been provided with access to a record of your personal information and believe that the record is inaccurate or incomplete, please fill out our Record Correction form (*form opens and is completed and submitted directly through website).
You must attach one copy of valid government issued photo identification along with your request.
